By just adding a cost (instead of zero cost) you just eliminate the npubs that splirt out thousands or millions of DMs to random users, hoping to achieve something

Instead of having virtually zero cost to spam 1m users, if you have to collateralize those DMs with, say, 10 sats each. Then spamming at that scale would cost 10m sats for the 1m DMs, making spamming too expensive as vast majority of spams yield no results

You would still have some spammers targeting specific accounts, pay the sats and hope to get something out of them and then at least the receiver gets paid, but if I wanted to send a serious DM to someone and just have to pay 10 sats for it, I personally wouldn't care to get the 10 sats back (would see it as a small payment for some of their time to read my DM)

And if a user can adjust the barrier (amount of sats that has to accompany a DM from a random user) they can adjust it higher or lower, depending on how much they value their time to read DMs from random people

And if you get a DM from someone and want to continue the conversation you can just mark the account as such that you can continue to communicate without anyone sending sats
